Government Service Strengthening

LAMB seeks influence through the following considerations:

  • What policies and systems do we most want to and are most able to influence?
  • Who can we influence that have themselves the ability to influence policy?
  • How can we influence those that matter?

Policies and systems where we can contribute

Women

  • Facility delivery by skilled birth attendant, nurse, doctor
  • Training of the various skill levels of obstetric care    (System)
  • Social development by women’s groups.  Clinic oversight participation.

Women/ Pro-poor

  • Healthcare equity – rich/ poor
  • Healthcare financing – targeted subsidies

Pro-poor/ Appropriate care

  • Rational use of drugs
  • Appropriate level of technology
  • Staff mix – doctor/ nurse ratio
  • Nurse anesthetists
  • SBAs in the community, home versus facility
Systems

  • Integration:  mutually accountable community, health services, authorities
  • Continuum of care: household, clinic, hospital with training, support, referrals
  • Staff recruitment and retention.  (Rec committee, English-medium school, )
  • Protocol driven clinical services
  • Management    (Not innovative, but important)
  • Triage for severity of illness
  • Task shifting to Medical Assistants
  • Training Centre methods:  skills- and values-based
  • Community organization engagement
  • Regular teaching/ in-services for all staff.  Keeping up-to-date.
  • Nurse empowerment
  • Nurse-doctor teamwork
  • Hands on nursing, by local standards
  • Chaplaincy/ counseling/ spiritual care
